Many patients report experiencing symptoms such as dizziness, nausea, fatigue, anxiety, and 'brain zaps'—a sensation often described as an electrical jolt in the head—that can persist for months or even years after stopping medication. In severe cases, withdrawal can mimic a relapse of depression, leading to misdiagnosis and inappropriate restarting of medication.
The new approach emphasizes a far slower and more individualized tapering process—sometimes reducing doses by just 10% increments over many months. Some doctors are now using liquid formulations or custom-made capsules to allow for these tiny, gradual reductions. This 'hyperbolic tapering' aims to keep changes in brain chemistry below the threshold that triggers withdrawal symptoms.
This shift has been driven in part by advocacy from patient groups who have long argued their experiences of withdrawal were dismissed. Research into the neurobiology of antidepressants suggests that the brain adapts to the presence of the drug, and that abrupt or fast removal does not give the brain time to readjust. The new protocols are designed to match the drug's diminishing effect on brain receptors.
