Coroner finds supervision failures contributed to Justin Cordy's death in Canberra jail
Special magistrate says the 34-year-old had access to the materials he needed to die by suicide at the Alexander Maconochie Centre in February 2023
A coronial report has found that supervision failures at the Alexander Maconochie Centre contributed to the death of 34-year-old Justin Cordy, who died by suicide in his cell on February 26, 2023. Special magistrate Lucinda Wilson found Mr Cordy was not adequately observed and was given access to the materials he needed to die by suicide at the Canberra jail.
