Alder Hey adds X-ray escalation process after toddler's death
Trust says Eleanor's death 'should not have happened' as parents speak of 'unbearable' guilt
Alder Hey Children's NHS Foundation Trust has taken full responsibility for 24 failures in care that led to the death of 21-month-old Eleanor Hazel Aldred-Owen, who died after routine surgery for a birth defect. The trust said her death was tragic and should not have happened. Eleanor's father said the trust was compelled to admit gross neglect.
