An investigation is under way after more than 150 critical hospital referrals were discovered forgotten in cardboard boxes, where they had sat untouched for 16 months. The incident has resulted in potentially catastrophic delays for cancer patients, including at least one patient who has died.
The referrals, which were intended to direct patients to urgent specialist care, were apparently set aside and never processed. The lapse has raised serious questions about administrative systems and patient safety at the hospital involved.
